A large percentage of stroke survivors require post-acute care and rehabilitation, but there are multiple levels of care available, and it is often difficult to determine the most appropriate care for a given individual. In addition, factors, such as race, ethnicity, and geography are associated with disparities in post-acute care, and structural biases in healthcare need to be understood in order to correct them. This discussion will review the differences among the various levels of care, explore the evidence on selecting the most appropriate level of care, share data on disparities in post-acute care, and present alternative models of care that have been demonstrated to be effective outside of the US.
